Handle generator values larger than TEXT.

A scanned page can supply a generator value larger than 65,535 bytes. With strict SQL mode, MySQL rejects it, rolls back the entire batch, and the writer drops that batch. Use a sufficiently large column or bound the value before insertion. (dev.mysql.com)

Review comment at @internal/db/mysql.go at line 278:
Update the insert flow that binds r.Generator so values exceeding the current
TEXT limit are accepted without causing the batch to roll back: use a
sufficiently large database column or bound the value before insertion, keeping
the schema and write behavior consistent.

Remove NUL characters before inserting the generator.

A response can contain a NUL inside the generator attribute. The extractor passes it through, but PostgreSQL TEXT rejects NUL characters. One such result rolls back the entire batch, which the writer then drops. Normalize the value before insertion. (raw.githubusercontent.com)

Review comment at @internal/db/postgres.go at line 257:
Normalize r.Generator before inserting it into PostgreSQL by removing any NUL
characters, while leaving the other response fields and generator content
unchanged.

Preserve the CSV schema when resuming an older scan.

When a scan resumes after an upgrade, the CSV writer appends the new header and rows to the existing file. The old and new rows then have different column counts, so CSV readers can reject the file or misalign fields. Keep generator out of CSV, or migrate existing files before appending. Review comment at @runner/types.go at line 53:
Remove the CSV mapping from the Generator field while preserving its JSON,
Markdown, and mapstructure mappings, so resumed scans retain the existing CSV
column schema.
